How do I know if I'm eligble?
You will need to discuss this with your GP, however, below are some of the criteria.
One or more medical conditions present for at least 6 months (or.
Terminal condition(s).
Managed care needs being managed by a medical practitioner (other than a specialist or consultant physician)
How can I be considered for this?
A patient is considered to have complex care needs if they require ongoing care from a multidisciplinary team consisting of:
GP or prescribed medical practitioner,
At least 2 other health or care providers.
Do I still need to pay? How much does this cover?
This plan covers $60.35 (July 1, 2024) per session
Only applicable to our Standard, Extended and Extended Plus appointments.
(Short appt is below the 20min threshold).
The remainder must be paid - HICAPS cannot be used with this Medicare EPC.
How often can this be used?
This depends on the quantity supplied by your GP
Maximum is 5 per calendar year.
